It\u2019s used daily by the youngest children with asthma as a preventive medicine so their airways don\u2019t get so swollen that a trigger \u2014 a virus, cold air, pollen \u2014 could cause an asthma attack.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmce0004356i6b806run@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            As a toddler, before Pearl\u2019s son started it, she said he would cough all night and sometimes gasp for breath when he talked. With the medicine, she said, his asthma was finally under control.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcf0005356iei9vk1da@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            But the maker of Flovent, British drug giant GSK, removed the branded drug from the US market in January, and its replacement \u2014 an authorized generic form identical in all but branding \u2014 doesn\u2019t have the same insurance coverage. It costs hundreds of dollars a month without it. The move has left families like Pearl\u2019s scrambling.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcf0006356iq6nwwyuv@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Asthma drugs can be pricy across the board, so much so that the Senate health committee, led by Sen. Bernie Sanders, opened an investigation into the situation in January.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcf0007356isrbtu6ct@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Shortly afterward, three of the biggest makers of asthma inhalers, including GSK, pledged to cap out-of-pocket costs for some US patients at $35.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcf0009356izdyl9mzc@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cWhile the progress that\u2019s been made with the out-of-pocket caps is great, and has left a lot of us in general feeling optimistic, children still can\u2019t get their hands on a preventive asthma inhaler,\u201d said Dr. Robyn Cohen, a pediatric pulmonologist at Boston Medical Center.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcf000a356ilebq53bh@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            It\u2019s a situation that has put kids in danger of asthma flare-ups and visits to the ER.    <\/p>\n<h2 class=\"subheader\" data-editable=\"text\" data-uri=\"cms.cnn.com\/_components\/subheader\/instances\/clv2orqsp0011356ipf5926lt@published\" data-component-name=\"subheader\" id=\"asthma-drugs-for-the-littlest-kids\" data-article-gutter=\"true\">    Asthma drugs for the littlest kids<\/h2>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcf000b356ihdkfj197@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            There are three inhalers in this category that the youngest kids with asthma can use, Cohen explained. They\u2019re called metered-dose inhalers, and they\u2019re paired with a device called a spacer \u2014 essentially a face mask that connects to the dispenser. That enables the medicine to be given as an aerosol so little kids can just breathe it in.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcf000c356ixqxks0g2@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cA child just needs to breathe through the face mask and we know that they\u2019re getting the medication,\u201d Cohen said. Other devices in the category, she explained, \u201crequire the patient to initiate a strong deep breath in, and then they have to hold their breath for 10 seconds.\u201d    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcf000d356iuhaamon4@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            That\u2019s not something<strong> <\/strong>toddlers and young children<strong> <\/strong>can be taught to do.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcf000e356idswtr9ou@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Since GSK took branded Flovent off the market earlier this year, the three options left have become increasingly expensive and hard to find.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcf000f356i52pgxakg@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            The authorized generic version of Flovent, fluticasone, is made by a different company, Prasco Laboratories. So GSK\u2019s pledge to cap monthly out-of-pocket costs at $35 across all of its asthma and COPD products doesn\u2019t apply to generic fluticasone.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcf000h356ia2l0eqdt@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Doctors like Cohen and Christy Sadreameli, a pediatric pulmonologist at Johns Hopkins, saw this coming last year. One solution was to try to switch kids to another option, Asmanex, but that drug has now gone into shortage as a result.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcg000j356ia7pgrf48@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            The third inhaler in this category is called Alvesco, and it also doesn\u2019t have broad insurance coverage, Cohen said \u2014 perhaps because it carries a higher list price. Sadreameli cited data showing it costs more than $300 a month before insurance coverage; Asmanex is less than half that.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcg000k356ipe71vmkz@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cAll of us know of know of a child who has struggled as a result of this,\u201d Cohen said.    <\/p>\n<h2 class=\"subheader\" data-editable=\"text\" data-uri=\"cms.cnn.com\/_components\/subheader\/instances\/clv2ou1n20013356ijsgfp06m@published\" data-component-name=\"subheader\" id=\"going-without-needed-medication\" data-article-gutter=\"true\">    Going without needed medication<\/h2>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcg000m356i0r107mcl@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Julie Leach\u2019s 13-year-old daughter, Abby, has a very rare condition called cerebrocostomandibular syndrome that restricted her lung development, and she was doing well on Flovent until it was discontinued.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcg000l356ixs014oqd@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            And the insurance tangles for these medications can lead families through bureaucratic odysseys to get the inhalers their kids need \u2014 a process they have to go through every month.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcg000n356i7yro0mar@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Her insurance suggested Abby use a drug called Pulmicort instead of the generic Flovent. Abby\u2019s doctor, Sadreameli, said Abby should stick with the metered dose inhalers that can be used with spacers, since Abby has a tracheostomy tube in her airway. Sadreameli and her team spent time working with Leach\u2019s insurance provider to agree on a solution, and they settled on Alvesco.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcg000p356i5i79s8to@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            It ended up taking a month to work out, while Abby didn\u2019t have any medication. When insurance finally did approve the drug, a patient assistance coupon was applied to it that carried a $65 copay, when Leach\u2019s secondary Medicaid coverage should have come with none.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcg000q356ivo4ycth4@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cIt took another probably two weeks for me to call around to all these people\u201d to get the coupon removed, Leach said. \u201cMeanwhile, she doesn\u2019t have any inhaler \u2026 I\u2019m panicking because she\u2019s never really been without an inhaler ever. So I really didn\u2019t know how her lungs were going to react, not having medicine.\u201d    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcg000r356ixwc29cfv@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Leach said every month, when she goes to fill the prescription \u2014 for just one of more than a dozen medicines Abby needs \u2014 she holds her breath.    <\/p>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv351e71000d356irp3ih8z4@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Yet it is not always clear which PBM a patient has when a doctor is prescribing a medicine, Sadreameli said, and they frequently encounter hurdles. And, said Cohen, out-of-pocket costs can still be high even with coverage.    <\/p>\n<div data-uri=\"cms.cnn.com\/_components\/factbox\/instances\/clv2pqttr00033b6iadhkw0pt@published\" class=\"factbox_inline-small         factbox_inline-small__standard  \" data-article-gutter=\"true\">\n<ul class=\"factbox_inline-small__items factbox_inline-small__items--ul\">  <\/ul>\n<\/div>\n<p class=\"paragraph inline-placeholder\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/clv2opmcg000s356i3gzuv0re@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Alvesco and Asmanex are made by Covis Pharma and Organon, which are not among the companies that made the $35 pledges on asthma and COPD drugs.
