Whooping cough is surging in the United States, with cases now five times higher than they were at the same point last year, according to preliminary data from the US Centers for Disease Control and Prevention that was reported Friday.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi00062e6b4jiylfcx@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            It\u2019s the highest number of whooping cough infections since 2014, \u201cwith no indication of slowing down,\u201d said Dr. Susan Hariri of the CDC\u2019s National Center for Immunization and Respiratory Diseases, who presented the data Friday at a meeting of experts who advise the US Food and Drug Administration on its vaccine decisions.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi00072e6bo4erql11@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            This year\u2019s spike in cases marks a return to pre-pandemic levels of this infection, which is also called pertussis.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b7eau000053b6ln4eob8ev@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Even before the Covid-19 pandemic, whooping cough cases were trending upward in the US, because the vaccines that are currently used offer strong initial protection that wears off after two or three years.    <\/p>\n<div data-uri=\"cms.cnn.com\/_components\/graphic\/instances\/cm1b6fubt00182e6b2fupfvx1@published\" data-component-name=\"graphic\" class=\"graphic\">\n<div id=\"graphic-6kcwm\" class=\"graphic__chart-anchor\"><\/div>\n<\/div>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi00082e6bc7xnjg43@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cWe know that we do not have very good pertussis vaccines,\u201d said Dr. Archana Chatterjee, a pediatric disease specialist and dean of the Chicago Medical School, at the meeting.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi00092e6bmfgshwon@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cThese vaccines have done a yeoman\u2019s job for us for many decades, but they are not doing the job as well as they used to do,\u201d said Chatterjee, who is a member of the FDA\u2019s Vaccines and Related Biological Products Advisory Committee.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000a2e6bufn6yfdi@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            On Friday, the committee met to discuss whether it might be time to allow pharmaceutical companies to purposely give people whooping cough as the primary test of their vaccines, a type of study called a human challenge trial.    <\/p>\n<h2 class=\"subheader inline-placeholder\" data-editable=\"text\" data-uri=\"cms.cnn.com\/_components\/subheader\/instances\/cm1b6kv6t001c2e6bn92j52mf@published\" data-component-name=\"subheader\" id=\"whooping-cough-cases-climbing-for-decades\" data-article-gutter=\"true\">        Whooping cough cases climbing for decades<\/h2>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000b2e6bi8xtkmg2@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            As with many infectious diseases, cases of whooping cough dropped to unusually low levels during the pandemic as people limited social interactions and took other precautions to defend against Covid-19.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000c2e6bdjk3ocnn@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            In 2020, the US had about 6,000 cases of pertussis, and that number dropped even further in 2021 to roughly 2,000 cases.&nbsp;There were about 3,000 cases in 2022 and about 5,000 last year.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000d2e6bvorjvyn7@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            The US has seen 14,569 cases this year, compared with 2,844 at the same time last year, according to the CDC\u2019s Nationally Notifiable Diseases Surveillance System.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000e2e6bfmuy9jh0@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Whooping cough starts with symptoms that look a lot like a regular cold: a runny nose, sneezing, a low-grade fever and a tickly cough. A week or two later, the infection moves into a new stage, leading to a violent full-body cough.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000f2e6buaomxu41@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            These coughing fits can be so severe that they cause patients to vomit or break ribs, and they\u2019re often accompanied by a whooping sound as the person tries to catch their breath.&nbsp;Antibiotics can treat the infection, but only if it\u2019s caught within the first few weeks, before the arrival of the exhausting, painful cough. Then, the only treatment is comfort care with plenty of rest and fluids while the infection runs its course.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000g2e6bpxpt1u37@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Unvaccinated babies and adults are those at highest risk of severe infection and, rarely, death. But doctors have increasingly been seeing vaccinated people with infections, too.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000h2e6bvd3pmnt8@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Young children typically receive five doses of a vaccine against pertussis \u2013 called DTaP or TdaP, which also protects against tetanus and diphtheria \u2013 by the time they\u2019re 6 years old.&nbsp;Boosters are recommended for children who are 11 or 12. Pregnant women are also recommended to be vaccinated against the infection, and adults are supposed to get boosters every 10 years.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000i2e6bx2fr7gqc@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Pertussis vaccination rates are high in the United States \u2013 around 90% \u2013 and did not dip during the pandemic, Hariri said, as they did in some other countries.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000j2e6bfwknaqbl@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Despite that, the upward trend in infections has continued.    <\/p>\n<h2 class=\"subheader inline-placeholder\" data-editable=\"text\" data-uri=\"cms.cnn.com\/_components\/subheader\/instances\/cm1b6q04a001h2e6bcfpy83mh@published\" data-component-name=\"subheader\" id=\"protection-from-vaccines-is-waning\" data-article-gutter=\"true\">        Protection from vaccines is waning<\/h2>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000k2e6bm65o7kvb@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            The increase in cases is not fully understood. There\u2019s evidence that it may be due to changes in the bacteria that causes the infection, said Dr. Tod Merkel, associate director of research in the FDA\u2019s Office of Vaccines Research and Review, in a presentation to the committee Friday.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000l2e6bdnn0hm61@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            In the 1990s, the US changed the type of vaccines used to prevent pertussis, moving from a whole-cell vaccine that caused children to spike fevers and experience other worrisome symptoms after their shots to a newer vaccine that didn\u2019t cause as many reactions but also offered less protection that didn\u2019t last as long.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000m2e6btpeq6yhz@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Other countries that changed to the newer vaccines have seen cases increase too, Merkel said.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000n2e6birt435tv@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            The incomplete protection from the newer vaccines, experts believe, has allowed the infection to continue to spread. It\u2019s also caused the bacteria to mutate to escape the immune response generated by the shots.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000o2e6b6b0k2th3@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Merkel said Friday that 90% of bacterial strains cultured in human infections in the United States are now those associated with mutations to skirt the protections of the vaccine.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000p2e6bmrj2ruaj@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            For young children who are vaccinated against pertussis, protection now wanes quickly, sometimes after two or three years \u2013 well before age 11, when a booster is recommended.    <\/p>\n<h2 class=\"subheader inline-placeholder\" data-editable=\"text\" data-uri=\"cms.cnn.com\/_components\/subheader\/instances\/cm1b6tcbq001j2e6b6kwus33k@published\" data-component-name=\"subheader\" id=\"regulators-consider-human-challenge-studies\" data-article-gutter=\"true\">        Regulators consider human challenge studies<\/h2>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000q2e6bpjrph8f8@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Although babies are most at risk from severe disease, infections are rising in older children and teens as protection wears off.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000r2e6b028193dj@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            That\u2019s led to a hunt for better, more durable immunizations against whooping cough that also don\u2019t cause as many side effects.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000s2e6blr9nvzfh@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Tests of new vaccines have been difficult, however, because whooping cough cycles, with major outbreaks every three to five years, and researchers can\u2019t predict where or when it will surge.&nbsp;Manufacturers need large numbers of infected people to test their inoculations.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000t2e6bqqmdm71q@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Enter human challenge studies.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000u2e6bzg1zhhlo@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            In some ways, whooping cough is an ideal infection to test in human challenge models. It\u2019s curable with antibiotics in its early stages, which protects study volunteers from harm.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000v2e6biihx080k@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            On the other hand, most people have few symptoms in those early stages.&nbsp;Investigators can measure the vaccine\u2019s ability to prevent mild early symptoms and whether it can prevent bacteria from setting up shop in nasal passages, but the pertussis challenge models don\u2019t allow them to directly measure how well a vaccine may prevent severe disease, which is the goal of most vaccines.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000w2e6bh35egpo8@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Researchers also don\u2019t know what immune factors may indicate that a person is protected from whooping cough, so they can\u2019t measure whether people are getting protection that\u2019s at least as good as that provided by the approved vaccines, a strategy called bridging.    <\/p>\n<div data-uri=\"cms.cnn.com\/_components\/factbox\/instances\/cm1b6dzk000132e6bxcr2r5x7@published\" data-component-name=\"factbox\" data-article-gutter=\"true\" class=\"factbox_inline-small factbox_inline-small__standard\">\n<ul data-editable=\"items\" class=\"factbox_inline-small__items factbox_inline-small__items--ul\">\n<li data-editable=\"items.0.text\" class=\"factbox_inline-small__item inline-placeholder\">Sign up here to get <strong>The Results Are In with Dr. Sanjay Gupta<\/strong> every Friday from the CNN Health team.<\/li>\n<ul><\/ul>\n<\/ul>\n<\/div>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000x2e6bca6uf0ub@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            On Friday, labs in Canada and the UK presented their models for how to conduct challenge trials for pertussis.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000y2e6b41t7wham@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            The FDA advisers were then asked whether they thought these kinds of trials could measure the right things and, if so, if they would provide enough evidence to be the basis of a vaccine approval.    <\/p>\n<p class=\"paragraph inline-placeholder vossi-paragraph-primary-core-light\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cm1b6d1zi000z2e6bmcbhhgrz@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            The labs\u2019 models were met with mixed reactions from the experts. On the one hand, they said, this approach was exciting and would surely speed the testing of new vaccines.
