wasn\u2019t spotted sooner.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5f00083b6mjg8u02o2@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            As president, Biden had access to VIP care, which goes above and beyond the health testing and health care offered to average patients. Members of the White House compound have access to medical staff 24 hours a day through the White House Medical Unit and special facilities are set aside for them at military hospitals. Presidential physicals typically leave no stone unturned when it comes to physical health and fitness for office, and presidents, including Biden, typically share some information from their medical checkups.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g00093b6malzzkhxz@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            So how could an 82-year-old<strong> <\/strong>president who\u2019s been out of office for only a few months have stage IV cancer that has spread to his bones?    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000a3b6mqo7xgm9k@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Experts say they understand that the circumstances might prompt fear and suspicion, but they\u2019re also not unheard-of. And although many men are familiar with what prostate cancer screening entails, some might not realize that typical screening is no longer recommended once men reach Biden\u2019s age.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000b3b6mors3igns@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Dr. Oliver Sartor, a prostate cancer researcher at Tulane University, said he\u2019s already fielded calls about Biden\u2019s diagnosis.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000c3b6m021y5p5i@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cI\u2019ve already been contacted by a friend of mine. He said, \u2018How could this possibly have happened? And it must have been a cover-up,\u2019 \u201d Sartor said, \u201cAnd I said, \u2018No, no, no, not necessarily. He\u2019d get up, he\u2019d put in a full day, and then at night, I\u2019d be in bed reading my book, and he was still on the phone, reading his briefings, working with staff.\u201d    <\/p>\n<h2 class=\"subheader inline-placeholder subheader-elevate\" data-editable=\"text\" data-uri=\"cms.cnn.com\/_components\/subheader\/instances\/cmavumo5400033b6n40xrb85s@published\" data-component-name=\"subheader\" id=\"screening-for-prostate-cancer\" data-article-gutter=\"true\">        Screening for prostate cancer<\/h2>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000i3b6m925xdnqw@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Prostate cancer is a significant health concern for men. It is the second leading cause of cancer death in American males, killing about 35,000 each year, according to the American Cancer Society.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000j3b6mjtnncd9n@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Among recent presidents, George W. Bush, Barack Obama and Donald Trump have had prostate cancer screening results reported as part of their annual physicals. Biden\u2019s medical reports during his presidency did not mention prostate cancer testing.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000k3b6myl0koe4e@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            President Trump said Monday that he was surprised that \u201cthe public wasn\u2019t notified a long time ago\u201d about Biden\u2019s diagnosis.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000l3b6m9e30yegz@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cI just had my physical. You saw that? You saw the results of that particular test.  I think the test is pretty much standard to anybody getting a physical,\u201d Trump said during a news briefing.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000m3b6m70r5melq@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Prostate cancer specialists say that\u2019s not necessarily the case, however.    <\/p>\n<div data-uri=\"cms.cnn.com\/_components\/graphic\/instances\/cmavnqinh001k3b6mk5176xq2@published\" data-component-name=\"graphic\" class=\"graphic-elevate\">\n<div id=\"cms.cnn.com\/_components\/graphic\/instances\/cmavnqinh001k3b6mk5176xq2@published\" class=\"graphic__anchor\" data-url=\"https:\/\/ix.cnn.io\/dailygraphics\/graphics\/20250519-sanjay-biden-cancer-callout\/index.html\" data-type=\"dailygraphics\" data-slug=\"20250519-sanjay-biden-cancer-callout\" data-pym-src=\"https:\/\/ix.cnn.io\/dailygraphics\/graphics\/20250519-sanjay-biden-cancer-callout\/index.html\"><\/div>\n<\/p>\n<\/div>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000n3b6mwt31h658@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Dr. Bilal Siddiqui, a cancer oncologist and prostate cancer researcher at MD Anderson Cancer Center in Houston, said there are two potential explanations for Biden\u2019s late-stage diagnosis.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000o3b6mza7bh1fs@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cThe first is actually the possibility that he wasn\u2019t being screened, which would actually be reasonable,\u201d he said.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000p3b6mb5k3d7rr@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Since the 1990s, men have been able to get a blood test to check for levels of a protein called prostate-specific antigen, or PSA, that might indicate the presence of prostate cancer. But the test has a high rate of false-positive results, up to 70%, according to the American Urological Association. Someone whose results show elevated levels may need a biopsy to rule out cancer.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000q3b6mjotoaycs@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Many medical associations say that whether to have the test should be a shared decision between a man and his doctor. Even then, screening isn\u2019t recommended for men of all ages.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000r3b6mrkm9fkwy@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            The majority of prostate cancers are slow-growing. In some cases, treatment can actually do more damage than the disease itself. Common side effects may include incontinence and erectile dysfunction. For that reason, the independent experts who review screening studies for the US Preventive Services Task Force recommend against screening for men 70 and older. Similarly, the American Cancer Society says men without symptoms who have a life expectancy of less than 10 years should not be screened. Biden is 82 years old, and the average life expectancy for a man in the US is about 76.    <\/p>\n<h2 class=\"subheader inline-placeholder subheader-elevate\" data-editable=\"text\" data-uri=\"cms.cnn.com\/_components\/subheader\/instances\/cmavuo98z00053b6nywyjdlks@published\" data-component-name=\"subheader\" id=\"not-all-prostate-cancers-are-the-same\" data-article-gutter=\"true\">        Not all prostate cancers are the same<\/h2>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000s3b6m42nunv6d@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Another possibility is that Biden had an \u201cinterval cancer,\u201d meaning it presented itself in the interval between regular screenings.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000t3b6mur327brn@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            Interval cancers are aggressive and can progress quickly, Siddiqui said.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000u3b6ml3zn70xp@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cIt\u2019s certainly possible that even within a six-month or a one-year period between screening, the cancer can grow fast enough to emerge as a metastatic cancer,\u201d he said.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000v3b6mkp3dg6un@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            In particularly rare cases, the cancer may not even produce PSA or might make very low levels that wouldn\u2019t raise alarm on a screening.    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmavnmr5g000w3b6mj6d0vip7@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">            \u201cThese are the most aggressive cancers, the ones that don\u2019t produce PSA, and so there are several possible explanations for why this diagnosis could have appeared,\u201d Siddiqui added.    <\/p>\n<div In nearly all men, the donut-shaped prostate swells with age, growing from the size of a plum to the size of a medium apple. In the process, it can choke off the urethra, the tube that empties the bladder. Men with BPH often have to get up in the middle of the night to pee and may have difficulty emptying their bladder.
